The following lists the document required for each participant application. Generate the full version with populated data through the user’s online panel.

#8 Official Transcript 2 is used to validate applicants who have previously attended another postsecondary institution prior to their current one stated in Part E (e.g., transfer student, postgraduate, etc.).If applicable, complete the fields and submit #8 Official Transcript 2 in Part F of the application panel.
Step 1 Enter information. For applicants who have previously attended another postsecondary institution prior to their current one stated in “Part E Current postsecondary institution,” complete the fields in “Part F Previous postsecondary institution” in the Participant Application panel. If not applicable, click the lock button above the upload button #8 to lock the fields in the section.

Step 2 Gather proof. Gather a qualified school-issued/endorsed document that displays information which allows FUSIA to verify information in the section. Ensure that it consists of the applicant’s name, school name, field(s) of study or concentration/specialization, degree or equivalent certificate, and most importantly, the admission and graduation (or end) dates.

Step 3 Submit proof. Upload #8 Official Transcript 2 in PDF format via the application panel. If the file has more than one page, combine all pages in one PDF and order the pages as per the following sequence.
